Title says it all. I'm currently looking for a period underwear that fits all these requirements:
1. Must be boyshorts or cover the entire buttcheek if not (idk why women's underwear companies don't do this 🙄)
2. Be for super heavy flow
3. Don't leak at night time
4. Not Thinx or any other companies with cancer chemicals
5. 100% cotton (idc if your recommendation is like 10% polyester tho)

These ones are 100% cotton disposable: https://a.co/d/7E8c8O1 but I think they will get more expensive than pads, which is what I've been using. Please help, thanks.

I use WUKA, a UK brand that has been independently tested and found to be PFAS free. They do short- style designs in heavy flow too which is really helpful.
Like you, I am sick of female underwear that doesn't have basic coverage or is uncomfortable just for the sake of being uncomfortable. So far, WUKA has worked really well for me!
Other PFAS free alternatives I've seen are modibodi and Bambody, so you could try them too.
If I'm going to be out all day or it's like the first day of my cycle, I use a reusable pad over the period pants that I then remove around lunch time. I bought a set of five or six from Amazon that came with a reusable waterproof bag that has two sections. There are a lot of similar options around, although I'm not sure how many have been tested for PFAS so I'm still looking into that!
I have been exclusively using period panties for the last 3 years. I mostly use the brand Bambody. It's made from bamboo.
I would recommend being careful of products that are 100% cotton because they will likely not be leak proof. Most period underwear has a plastic-like lining that creates the leak proof barrier.
I also bought a pack of period underwear from Amazon called Neione by Intimate Portal. They are bikini cut, for light flow days.
I have a moderate flow in this works for me. However, if you have a heavy flow, I would recommend using a menstrual cup in addition to the underwear. I would also recommend a reusable pad, I use one for heavy days.
I understand you have concerns about PFAS. Personally I don't worry about it much. There are so many cancer causing chemicals used in everyday products (plastic Tupperware, non-stick pans, etc.). And disposable period products have concentrated levels of pesticides from the low quality cotton used in them.

I don’t have any advice for any type of panties, but I’ve also had a heavy flow since I was younger. Two years ago, I found out I was anemic due to heavy bleeding. I’ve been on iron pills and have gotten iron infusions to help my iron and hemoglobin levels. About a week ago, after I did a blood work check I found out that I have a disease called Von Willebrand disease. I lack a protein that helps clot blood, which is what makes me bleed excessively and I have type 1 so it’s inherited. If possible, I recommend talking to a hematologist and getting blood work done if you don’t know the cause of your heavy bleeding. I’m also on oral birth control and Depo for my bleeding, but it only helps a little bit so I’m planning to get an IUD. Hope this helps you
Don’t know if you can get Bonds Bloody Comfy underpants outside of Australia, but they’re my choice. All cotton, and the absorptive gusset is also cotton topped.
I'd be interested in this too, or maybe some reusable types of pads that I could buy. I'm using tampons and what not but I'd rather go with something more sustainable.
